The following Terms and Conditions (“Terms”) govern participation in The Ultimate Experience VIP Membership Program (“Membership Program”) offered by Ultimate Image Cosmetic Medical Center (“UICMC”). By enrolling in any tier of the Membership Program, clients agree to the Terms outlined below.
GENERAL MEMBERSHIP TERMS (APPLICABLE TO ALL TIERS)
1. Membership Eligibility
Memberships are available to clients age 18 and older with a valid photo ID and active payment method on file.
2. Activation Date
All memberships activate on the date of purchase unless otherwise stated during promotional presale periods.
3. Membership Term Commitment
Each membership tier includes a minimum commitment period:
Members are financially responsible for all payments within the commitment period, regardless of usage.
4. Payment & Billing
Membership fees are billed monthly to the payment method on file unless the member elects to pay for the entire commitment period in advance.
Payments are non-refundable once processed.
Declined or expired cards must be updated within 72 hours to avoid disruption of benefits.
5. Appointment Cancellations & No-Shows
Standard UICMC appointment policies apply to all memberships. Missed or late-canceled appointments do not roll over or convert into credits.
6. Rollovers & Expirations
Membership benefits, perks, and monthly inclusions do not roll over unless explicitly stated.
Bonus items issued during promotions may include expiration dates.
7. Non-Transferability
Membership benefits, treatments, products, or credits cannot be gifted, transferred, or shared.
8. Member Discounts
Membership discounts apply to eligible treatments, services, and retail purchases. Discounts cannot be combined with other promotions unless explicitly stated.
9. Changes to Program
UICMC reserves the right to modify membership pricing, terms, or benefits with 30 days’ notice to active members.
10. Termination & Cancellation
Members may cancel after completing the minimum commitment period with 30 days’ written notice.
Early cancellation requests:
Require full payment of remaining commitment or may result in forfeiture of benefits already redeemed.
11. Loyalty Points
Points accrue at the rate assigned to each tier (1–3 points per $1).
Points may only be redeemed in increments described in the Loyalty Points Program.
If a membership is canceled, all unused loyalty points are forfeited.
TIER-SPECIFIC TERMS & CONDITIONS
Below are the formal conditions and inclusions applicable specifically to each membership tier. All tier-specific benefits must be used within the active membership period and do not roll over.
ESSENTIALS MEMBERSHIP – TERMS & CONDITIONS
Monthly Fee: $99/month
Commitment: 12 months
Included Benefits:
Earn 1 point per $1 spent on services and retail.
10% discount on skincare products.
One Deep Pore Facial ($89 value) during each membership year.
One $100 UICMC Gift Card issued annually during active membership.
One skincare product valued up to $100 per membership year.
Monthly B12/MIC Injection (non-transferable; must be used monthly).
Access to member-only specials and promotional pricing.
Priority booking with preferred providers (not guaranteed for same-day appointments).
Conditions Specific to Essentials Tier:
Monthly injection cannot roll over or be exchanged for value.
Deep Pore Facial and choice of product must be redeemed within 12 months of membership activation.
Early cancellation results in forfeiture of unused benefits.
SIGNATURE MEMBERSHIP – TERMS & CONDITIONS
Monthly Fee: $149/month
Commitment: 9 months
Included Benefits:
Earn 2 points per $1 spent.
15% discount on skincare products.
One Signature HydraFacial ($199 value) during each membership year.
Monthly B12/MIC Injection ($360 annual value).
One $150 UICMC Gift Card issued annually.
Choice of ONE of the following annually:
12 units Botox / Xeomin OR 24 units Dysport
Venus Velocity Laser Hair Removal (small area)
Skincare product valued up to $150
Conditions Specific to Signature Tier:
Monthly injection cannot roll over or be exchanged for value.
Signature HydraFacial and choice of service/product must be redeemed within 9 months of membership activation.
Laser and injectable selection may not be substituted.
Early cancellation results in forfeiture of unused benefits.
ELITE MEMBERSHIP – TERMS & CONDITIONS
Monthly Fee: $199/month
Commitment: 6 months
Included Benefits:
Earn 3 points per $1 spent.
20% discount on skincare products.
One Deluxe HydraFacial ($250 value) during each membership year.
Monthly B12/MIC Injection ($240 value).
One $200 UICMC Gift Card issued annually.
Choice of ONE annually:
24 units Botox / Xeomin OR 48 units Dysport
Venus Versa Pro Intense Pulse Light (IPL) Treatment of Choice - Face, Neck, or Decollete ($250 value)
Venus Velocity Laser Hair Removal (medium area)
Skincare product valued up to $200
Conditions Specific to Elite Tier:
Monthly injection cannot roll over or be exchanged for value.
Deluxe HydraFacial and choice of service/product must be redeemed within 6 months of membership activation.
Laser, injectable, and IPL selection may not be substituted.
Early cancellation results in forfeiture of unused benefits.
LOYALTY POINTS TERMS & CONDITIONS

Redeeming Points
Points may be redeemed only in increments approved by UICMC (e.g., 500 = $25 off, 1000 = $50 off, etc.).
Maximum redemption per transaction is 5,000 points ($250 off).
Expiration & Forfeiture
Points expire if membership is canceled or if no activity occurs for 12 months.
Points cannot be exchanged for cash, gift cards, or transferred to another member.
